PRN Bilingual Mobile Outreach RN

Empath Hospice, a member of Empath Health is currently seeking a dedicated community outreach Registered Nurse (RN) to join our team. The ideal candidate will be passionate about advocating for individuals and families within the community, providing support, resources, and services to enhance their well-being.

The Care Navigation Mobile Outreach Registered Nurse (RN) is responsible for coordinating service inquiries as well as offering prompt access within the Empath Health Network of Care for targeted communities via a community outreach vehicle. Maintains excellence in customer service by being caring, compassionate, empathetic, and responsive to the needs of all who reach out to us. Serve as a valuable participant on committees within our community hospitals. Embracing the Empath mission, vision and values while advocating for optimal access to the right care at the right time in any care setting.

Position Requirements :

• Education and / or Experience : Associate’s degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and / or training; or equivalent combination of education and experience.
• Licensure :

Valid Florida license as a Registered Nurse
