Overview
We are seeking a dedicated and organized Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that clients receive the highest quality of care by coordinating their care services effectively. This position requires strong communication skills, attention to detail, and a solid understanding of in-home senior care. Experience in a private duty home care is required.
Responsibilities
- Serve as the primary point of contact for clients, addressing inquiries and providing information about services.
- Coordinate client appointments, ensuring timely scheduling and follow-ups.
- Maintain accurate cllient records and documentation in compliance with privacy regulations.
- Collaborate with healthcare professionals to facilitate client care as needed.
- Assist with administrative tasks such as filing, data entry, and managing office supplies.
- Monitor clients care and communicate updates to relevant family and / or healthcare providers.
- Provide caregiving when needed.
- Answer calls after hours from current clientele or potential new clients and the caregiving staff to ensure effective communication.
- Compliance with state regulations.
Qualifications
Previous experience in a home care office is highly desirable.Previous caregiving experience is required.Familiarity with home care and caregiving terminology and administrative support functions is essential.Strong organizational skills with the ability to manage multiple tasks efficiently.Excellent verbal and written communication skills to interact effectively with clients and staff.Proficient in using office software and electronic health record systems.Ability to work independently as well as part of a collaborative team environment.Join us in making a difference in our patients' lives by providing exceptional care coordination!
