Your Role :

Manage and oversee patient care plans

Collaborate with healthcare teams

Educate patients and families

Perform assessments & adjust care plans

Ensure accurate documentation

What You Need : Active RN license

1+ year of clinical nursing experience (home health preferred)

Strong assessment & communication skills

Ability to manage a caseload independently
