POSITION SUMMARY : Perform the duties required to provide services to refugees by promoting access to preventive healthcare services, health education, PRAPARE screenings, follow up, and linkage and navigation services for resources to support wellbeing and healthy lifestyles.
POSITION QUALIFICATION REQUIREMENTS
Education : High school graduate or equivalent. Certified Nurse Assistant (CNA) and / or Certified Community Health Worker (CHW) preferred. Fluency in English and another language (Farsi / Dari, Pashto, Swahili, Spanish, or Arabic) preferred.
Experience :
- Two (2) years working in a community health setting, public health, or similar setting.
- Experience in collaborating with refugees, immigrants, and / or other similar diverse populations.

Strong decision making and prioritization skills.Ability to work with minimal supervision.Requires good written communication and organizational skills.Strong attention to detail : must be highly flexible, and able to keep multiple assignments moving forward with a focus on continuous improvement.Communicates effectively with health center team and clients.Exhibits teamwork mentality.Good computer skills including the use of word processing and e-mail software.Transportation Requirement : Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri .
License(s) / Certification(s) Required : CPR certification. Must attend a 16-20-week Community Health Worker (CHW) course as soon as possible upon hire, unless already a Certified CHW.
FUNCTIONS OF THE JOB
Essential Functions
Schedules and meets with patients to assess their needs, and available support.Provides patients with health information, health education and materials in their preferred language.Provides patients with education on how to access health care services including healthcare appointments, prescriptions, labs, urgent care, and other health care services.Schedules healthcare appointments and provides reminders and follow up support for missed appointments.Recruit patients for, schedule, and facilitate refugee health education groups.Complete PRAPARE screeners to assess social determinant of health barriers and needs.Provide navigation to remove barriers to care such as transportation access and routes, planning for arrival times, paperwork completion for healthcare appointments, interpreter services, what to bring to a healthcare appointment, and other planning, information and direct supports needed.Help patients access application assistance for Medicaid and other benefit programs, such as WIC, for those who are eligible.Responsible for filing appropriate documentation in patients' Affinia Healthcare electronic health record.Facilitate and assist with community referrals.Enter patient care information on Client Track, NextGen, and department spreadsheets as directed by supervisor.Other duties as assigned.Marginal Functions
