Position Highlights & Primary Responsibilities :
- Build and nurture relationships with patients, caregivers, and interdisciplinary team members.
- Conduct comprehensive assessments for newly enrolled patients and provide continuous care.
- Create and regularly update patient management care plans based on health changes and hospitalizations.
- Consult with Physician Leaders and coordinate care with Primary Care Providers as needed.
- Perform essential nursing procedures, including IV placement, blood draws, and basic wound care.
- Conduct post-discharge visits to ensure patient stability and alignment with care goals.
- Engage in quality initiatives to improve healthcare standards and delivery.
- Participate in an after-hours on-call program on a rotational basis.

Required Qualifications :
Certification from the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).Current, unrestricted Nurse Practitioner and RN licensure in Missouri.A minimum of 2 years of clinical experience in primary care, geriatrics, or home-based medical care.Reliable transportation for conducting in-home visits throughout the state.Preferred Qualifications :
Knowledge of long-term care and community resources for patient care.Familiarity with clinical standards, CPT, ICD-10, and HCPC coding.Understanding of NCQA requirements, CMS guidelines, and Medicaid / Medicare systems.The work you do will change lives.
