Position Highlights & Primary Responsibilities :

• Build and nurture relationships with patients, caregivers, and interdisciplinary team members.
• Conduct comprehensive assessments for newly enrolled patients and provide continuous care.
• Create and regularly update patient management care plans based on health changes and hospitalizations.
• Consult with Physician Leaders and coordinate care with Primary Care Providers as needed.
• Perform essential nursing procedures, including IV placement, blood draws, and basic wound care.
• Conduct post-discharge visits to ensure patient stability and alignment with care goals.
• Engage in quality initiatives to improve healthcare standards and delivery.
• Participate in an after-hours on-call program on a rotational basis.
