Advanced Practice Provider-Primary Care At Home

A diligent and compassionate job-seeker is needed for the Nurse Practitioner opportunity with our company! As the Nurse Practitioner, you will be responsible for providing consultation, follow-up, and face to face visits to patients in multiple environments. This opportunity offers $108k-$135k annually based on experience and potential bonus incentives based upon Productivity and Performance goals.

What You'll Do :

• Perform follow up visit to patients in home settings and facilities
• Provide a health assessment appropriate to the visit being performed, as well as diagnose and treat patients who require management of symptoms
• Provide referrals / orders for lab work, testing, DME, homecare, PT, OT, etc. as appropriate
• Refer patients to other Samaritan services as appropriate
• Provide appropriate communication with patient's specialists
• Complete documentation in EMR in timely manner and according to Samaritan policies
• Represent Samaritan in a positive manner to patients, families and the community
• Support and participates in performance improvement and quality assurance programs.
• Attend staff meetings and training courses / classes as required.
• Provide on-call coverage on rotating basis
• Other responsibilities as assigned
