Hospice RN Case Manager, Full-time

Transitions LifeCare seeks to hire full-time Community-based Hospice RN Case Managers who embrace our mission of caring for individuals and families with compassion, support, and expertise as they navigate the end of life or life-changing illness.

This is an exciting time to join our team! Qualifying Experienced RNs with at least (1+ years of experience) may be eligible for up to a $20,000 Sign-On

The schedule for this position is Monday-Friday 8 : 00am-5 : 00pm; available one weekend shift per quarter; one major or two minor holidays per year.
