The Registered Nurse-Community Care Coordinator serves as a liaison to both internal and community providers and is responsible for managing the care of the Veteran throughout the consult process. Supports the medical center's mission to improve timely access to care, ensure Veterans who want to receive care in the community are referred and scheduled into the community, and support the delivery of high quality, timely care to the Veteran receiving community services.
Duties include but not limited to :
Assists the Nurse Manager in (1) Planning work, setting priorities and preparing schedules for completion of work; (2) Assigning work based on priorities, selective consideration of the difficulty and the requirements of the assignments, and the capabilities of staff (3) Evaluating the performance of staff; (4) Making recommendations for appointments, advancements or reassignments of staff; (5) Giving advice, counsel, or instruction to personnel on work and administrative matters; (6) Hearing and resolving complaints; (7) Addressing employee conduct and performance issues; and (8) Identifying developmental and training needs of staff and providing or making provision for such development and training).
Works with the Nurse Manager to comply with regulatory standards and maintain continuous accreditation readiness of the unit. Will assist with performance improvement data collection, customer service / patient satisfaction, patient and employee safety, interviewing applicants, employee relations, and standards of care and practice. Actively develops and manages collaborative partnerships with physicians, nurses, other clinicians, and administrative staff both within the VA and the community. Supports the Nurse Manager in staffing the unit on a continual basis. Participates in service and medical center committees and task forces to improve nursing practice and patient care. Has an integral role in orienting new nurses.
