Urgent Care Nurse Practitioner

Job Title : Nurse Practitioner ( New graduates welcome

Location : Hartford, C T

Responsibilities :

Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.

Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident.

Manage chronic conditions and acute illnesses, providing timely interventions and appropriate referrals as needed.

Order and interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning.

Prescribe medications and treatments within the scope of practice and in accordance with state regulations and facility policies.

Participate in care initiation for our chronic care management services.

Requirements

Graduate of an accredited Nurse Practitioner program

Licensed or eligible for licensure in Connecticut

Strong clinical knowledge and decision-making skills

Excellent communication & cultural competence

Comfortable with Electronic Health Records (EHR)

Eligible for DEA and state narcotic registration

1-2 years' experience in ambulatory / urgent care preferred
