We are seeking a Nurse Navigator to coordinate care for the high risk, chronically ill patient across the continuum, and to promote effective education, self-management support and timely healthcare delivery to achieve optimal quality and financial outcomes.

REQUIREMENTS : Graduate of an accredited school of nursing, Bachelor's Degree preferred. Current RN license in the State of Massachusetts. Three to five years of clinical related job experience, case management experience preferred.
