Planet Healthcare is seeking a travel nurse RN Home Health for a travel nursing job in New Lenox, Illinois.

Job Description & Requirements

• 40 hours per week
• Shift : 8 hours, days
• Employment Type : Travel

EMR : Matrix - will be provided an iPad.

Interview Process : Phone screen and in person or virtual interview with the clinical manager.

Territory :  Within 15 miles (may increase if census is low and they need to offer the 40 hours)

Census :  24-26 on case load. If given a bigger territory that number will go down.

Mileage : Reimbursed at the standard IRS rate.

Perform some Telehealth / Virtual visits in addition to home visits

Orientation :  2 days. First day will be in the office. Second day out in the field.Services :

Catheter Care

Chronic Disease Management & Telemonitoring

Diabetes Management

Home Health Aide Services

Infusion Management (including antibiotic therapy)

Laboratory testing

Medical Social Service

Nutrition Instruction

Physical, Occupational, and Speech Therapy

Skilled Nursing

Telemonitoring and Virtual visits

Tube Feeding

Wound / Ostomy Care
