About the Team :

The Skilled Rehab and Home Health team is eager to welcome a Physical Therapy Assistant to our dedicated staff. This full-time position (35-40 hours per week) offers flexibility for weekdays, weekends, and holidays and involves providing care to clients in both our Davenport Skilled Nursing Facility and in home health settings. Our team treats various conditions, including orthopedic issues, weakness, balance / falls / gait, cervical / back pain, and general deconditioning. We currently have 2 full-time Physical Therapists and 2 full-time Physical Therapy Assistants. We warmly welcome new graduates to apply!

Key Responsibilities :

• Assist patients, residents, and clients with exercises and therapy activities as outlined in their treatment plans under the supervision of a licensed Physical Therapist.
• Monitor, record, and provide documentation of therapy services, ensuring alignment with established standards and protocols.
• Select and implement therapeutic activities that align with patient goals and practice standards, adjusting as necessary based on patient responses.
• Collaborate with the supervising Physical Therapist, sharing insights on patient progress and areas for improvement.
• Stay informed about the targeted physical therapy outcomes for each patient and document progress accordingly.
• Adhere to all clinical practice guidelines set forth by relevant professional organizations.
• Be prepared to provide care across post-acute, long-term care, and home health care environments as needed.

Qualifications :

• Graduation from an accredited Physical Therapist Assistant program recognized by the American Physical Therapy Association (APTA).
• Current licensure as a Physical Therapist Assistant in the applicable state.
• Basic Life Support (BLS) certification is required within six months of hire.
• A valid driver's license may be necessary, depending on the role.
