job_description.job_card.job_descriptionProvide, coordinate, and direct the provision of home nursing care according to physicians' orders, based on agency policies and procedures, through the competent application of the nursing process. Responsible to the Clinical Manager, directly supervising and evaluating Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses. Average of 5-8 patients per day, with a productivity expectation equivalent to 30 points per week. Types of visits include Start of Care, Wound Care, Follow-up Visits, etc. Complete all documentation within 24 hours of patient appointments. Attend periodic office meetings and manage home visits effectively. Participate in on-call rotations every other weekend and two days during the week. Travel mileage reimbursed at the IRS standard rate (commute to and from the first and last patient are not billable). Attend orientation and any required retraining courses during compliance if proficiency in HomeCare HomeBase or Oasis is not current. Must accept every patient on the schedule, regardless of the distance between appointments. Local candidates accepted at the same rate.

Experience : Minimum of 1 full year of Adult Home Health experience or 1 full year of Hospice experience.
License : Must have a valid license in hand before applying.
Certifications : CPR and BLS required
Must-Have :
- Proficiency in HomeCare HomeBase and Oasis documentation systems
- Reliable transportation, current driver's license, and agency-required liability insurance
- Ability to travel up to 75 miles from the home clinic
