If you are passionate about working with teens and young adults, this position could be right for you. As a Transition Facilitator, you will coach, support and help young adults (ages 16.5 to 22) with emotional, behavioral or mental health issues as they transition from the children’s behavioral health system of care into adulthood or to the adult system of care. You will provide guidance and support in the areas of employment, education, housing, social and family relationships, independent and daily living, finances and budgeting, decision making, emotional and physical health and wellbeing, community life, and more using principles of the nationally recognized Transition to Independence Process program.
You will engage as a member of a transition team to develop solution-focused strategies to overcome barriers. You will be responsible to evaluate youth to determine their suitability to enter the program, then work with them and facilitate service plans to progress them through the program until completion. You will work collaboratively with other service providers and educators from children’s mental health, education, adult mental health, child welfare, rehabilitation and other partners as needed to ensure appropriate support for each young adult. You will work in the community and at non-traditional hours, in the places and at the times that work best for these young adults. You will also be responsible for administrative and documentation functions.
