Caregiver Position

Summary : As a Certified Home Health Aide (HHA), you will provide care to clients in their home, following their plan of care. You make a positive difference each and every day by demonstrating care and compassion to our patients.

Responsibilities

• Patient Care & Service - Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse / Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands.
• Communication - Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly.
• Documentation - Accurately and completely document client care on activity sheet.
• Compliance - Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements.

Physical Requirements

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and / or 10 to 25 pounds of force frequently, and / or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

Pay Range

$18.65 - $19.75 The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
