WCS Healthcare is currently seeking an  Case Management Nurse for a contract assignment at a Prestigious Hospital in Northampton, MA.

Job Function :

Day Shift.

8am to 4 : 30pm.

Assess new patients.

Update and revise patient health care plans as needed.

Educate patients and families about health care plans and options.

Make clinical decisions for routine patient care.

Accurately document and submit medical records.

Demonstrate knowledge in medical care; such as diagnostic procedures, medication, symptoms, and other treatment-related therapies.

Job Requirements :

Graduate from an accredited School of Nursing or Bachelor's Degree in Nursing (BSN).

Current RN license for the state that you are working in (or willingness to obtain).

BLS certification from the American Heart Association.

CCM certification preferred.

2 years of acute care experience in this specialty.
