Community Health Care Coach

Salary : $39,000

Elder Options is seeking a Community Health Care Coach, to provide community health education, follow up support and coaching to patients. Services include Care Transition services supporting hospital to home transitions and / or other community-based health and wellness services. The interventions are geared toward patient self-management of chronic diseases via teaching / modeling, self-advocacy, and education with the purpose of reducing re-hospitalizations, providing individualized assessments, care planning, referrals and supports to adults with health and wellbeing goals to support their independence in the community. Home visits are required and remote work is limited.

ADA / EOE. Level 2 background screening required. Position requires regular in-office work, but may allow for remote work upon completion of training.
