Family First Healthcare Services in Florida is seeking a Clinical Liaison in home healthcare (Bi-Lingual in English and Spanish preferred) who is highly capable of building relationships to coordinate patient care transitions between different facilities and our agency. Key responsibilities include assessing and screening potential patients, coordinating admissions and transfers, and serving as a communication bridge between referring providers, patients, families, and our agency's clinical team.

Our Clinical Liaison will work in close concert with of our Head of Business Development & Growth. This person will also act as a marketer for our organization and provide education to patients and families.
Key responsibilities
Relationship building : Cultivate and maintain relationships with external referral sources, including hospitals, physicians, case managers, discharge teams, and Payors to increase patient referrals.
Patient and family education : Educate patients and their families about our agency's services and what to expect during their stay, providing comfort and support. Integrates evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of post-acute care.
Ability to review patient medical charts and understand test results, therapy evaluations, preexisting conditions, and have a general medical knowledge of the patient. Ability to clearly and professionally interact with patients, families, and healthcare providers while gathering additional clinical information and past history
Experience conducting thorough patient assessments to identify patients for potential admission into the rehabilitation program. Able to complete detailed Pre-Admission Screens, as applicable, according to agency's policies and procedures. Schedules meetings and arranges services for medical professionals including potential and existing referral sources, doctors, nurses, social workers and other health care professional.
Provide patient updates to physicians, payers, case managers, social workers and other relevant persons. Collaborate with caregivers and office staff to ensure seamless coordination of services.
Liaison and communication : Act as the primary point of contact, ensuring clear and consistent communication between the patient, their family, the referring sources, and Family First’s receiving clinical team. Conduct in-home or facility assessments to determine client needs and develop individualized care plans. Maintain solid working relationships with new and existing referral sources by providing excellent after-sales service.
Marketing and promotion : Promote the organization's services to potential referral sources and qualified patients and their families, sometimes by leading tours or giving presentations. Educate referral sources and community partners Family First’s available home healthcare and home care services options.
Ability to achieve and surpass established productivity metrics and enable the agency’s overarching mission and goals. Assist with follow up with clients and families to ensure satisfaction and adjust care plans as needed.
Actively participate in outreach events and community networking to strengthen referral relationships. The Clinical Liaison serves as the primary connection between referral sources, clients, families, and our home care team. This role focuses on client intake, coordination of care plans, communication between healthcare partners, and ensuring high-quality, client-centered service delivery.
