Physicians Choice Home Health & Hospice is looking for full time Certified Nursing Assistants (CNAs) to support the needs of our growing patient population in Las Vegas, NV

We are looking to fill a shift of Wednesday - Sunday

Our CNAs provide services for both home health care and hospice patients. Patients may be housed in group care homes, assisted living facilities, or private residences.

1. Performs duties as assigned by the DON or RN including baths, oral hygiene, nail care, foot care, skin care, pressure relief, dressing patient, feeding and transferring patient.

2. Notifies appropriate licensed personnel when patient complains of pain.

3. Takes accurate vital signs.

4. Responds to the physical, mental, social, and emotional needs of terminally ill patients and their families within scope of practice.

5. Describes the activities of daily living and demonstrates the ability to use techniques to assist the patient to meet these activities until no longer able.

6. Positions patient in bed with proper body alignment

7. Assists patient with ambulation until no longer able.

8. Identifies and communicates findings to the RN or DON including changes in pain and symptoms that they document in the record.

9. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical / health care errors.

10. Demonstrates the ability to chart according to professional standards.

11. Works as part of the Interdisciplinary Team.

12. Addresses and supports cultural practices if such practices do not harm others.

13. Adheres to dress code, appearance is neat and clean, wears identification while on duty.

14. Completes annual educational requirements.

15. Maintains regulatory requirements.

16. Always maintains patient confidentiality.

17. Reports to work on time and as scheduled, completes work within designated time.

18. Attends annual review and in-services.

19. Attends routine staff meetings as requested.

20. Complies with all organizational policies regarding ethical business practices.

21. Participates in Quality Assessment and Performance Improvement (QAPI) activities when requested.

22. Represents the organization in a positive and professional manner in the community.

23. Communicates the mission, ethics, vision, and goals of the organization.
