Reports to : Family Housing Services (FHS) Housing Supervisor
About the Department :
The Family Housing Services department is made up of housing for families with children, pregnant women and youth who are at-risk of becoming unhoused or unhoused living in shelters, vehicles or on the streets.
Job Summary :
The Operation Home Case Manager, reporting to FHS Housing Supervisor, provides direct case management services to clients receiving housing assistance in HELP's Operation Home Program.
- Conduct initial interview / intake and client needs assessments
- Provide case management and supportive services to clients participating in HELP housing programs
- Create individualized case plans with each adult member of the household, based on clients' needs, with the goal of achieving self-sufficiency
- Review and update client's case plan monthly, and track outcomes
- Meet with clients on a weekly basis
- Complete monthly rent requests and income calculation worksheets
- Conduct regular home visits
- Connect client with wrap-around services through other programs offered at HELP
- Connect client with mainstream programs, including but not limited to, Medicare, WIC, TANF, SSI / SSDI
- Provide client with referrals and resources for other social services and partnering agencies
- Enter case notes in true time data (daily) into HMIS, reflecting activities of the clients' progress
- Maintain current and accurate client files
- Maintain clear boundaries with all clients and providers
- Always maintain and protect both your personal and the organization's ethical standards, ensuring they are never compromised
- Provide transportation to clients to ensure appointments are kept
- Complete monthly report as per grant guidelines
- All other job-related duties as assigned by management for needs of the organization

Knowledge of local social service agencies and mainstream programsKnowledge of applicable statutes and regulationsKnowledge of social service work, interviewing / referral techniquesExperience and understanding of unhoused issuesUnderstand and ability to complete complex instructions and proceduresAnalyze situations accurately and adopt an effective course of actionCommunicate effectively with persons from a variety of social, cultural, and economic backgrounds, and possess a demonstrated understanding of and sensitivity to culturally diverse populations with effective written, verbal, and listening skillsAbility to maintain order and exercise appropriate judgment in crisis situationsAbility to work flexible hours and adjust to rapidly to shifting prioritiesAbility to organize work schedule and effectively manage timeAbility to understand and follow all agency policies and proceduresGeneral computer knowledgeAbility to perform accurate mathematical equations to calculate client's rent and utility cost, and to request funds to pay those billsRequired Qualifications :
Bachelors Degree in social work of related field; OR combination of education and experiencePrevious experience working with unhoused individuals, families, and childrenActive NV Drivers License and Clean Driver Vehicle ReportPreferred Qualifications :
Previous experience working in a social services setting
