Responsibilities

Provide one on one care to pediatric and young adult clients in their homes

Deliver skilled, compassionate support for complex medical needs

Monitor, assess, and document changes in condition

Follow the established plan of care and communicate with the care team

Build meaningful, lasting relationships with patients and families

Requirements

Active LPN or RN license

Pediatric experience preferred but not required, training provided

Commitment to patient centered care

Ability to meet all compliance and background requirements
