A company is looking for a LPN / LVN Health Coach.

Key Responsibilities

Provide remote patient care to individuals with chronic conditions using digital communication tools

Conduct onboarding and monthly engagement calls with patients to review care plans and address needs

Educate patients and families on home monitoring devices and coordinate care related to medications and therapies

Required Qualifications

Associates or equivalent degree required

Active multistate license as a practical nurse or vocational nurse in a compact state

2+ years of experience in a patient care role, preferably in a remote setting

Strong customer service skills and ability to manage multiple tasks in a dynamic environment

High-speed home internet access and a quiet home office space
