A Case Manager provides evaluation, consultation and intervention with youth and families within the community. Also participates in conducting psychosocial programming with this population as part of a clinical team.
EXAMPLES OF WORK PERFORMED :
- Works with youth and their families in school, mental health, community, home or office settings.
- Works with community and family for the purposes of creating and maintaining individualized service arrays for families.
- Provides case management and some skills training with children and families.
- Makes assessment and psychosocial evaluations and recommendations.
- Refers families of clients to appropriate social welfare and health agencies to obtain assistance.
- Performs Related Work as Required.
Experience and Training :
Experience as set forth under Education.Must have current valid driver’s license and current and continued proof of liability insurance.Case management experience preferred.Education :
Bachelor’s or Master's Degree in Psychology, Social Work or related field.
KNOWLEDGE, SKILLS AND ABILITIES :
Knowledge of emotional problems as well as rehabilitation / skills training and treatment approaches.Must have excellent verbal / written communication and knowledge to enter clinical documentation.Ability to establish and maintain positive working relationships with clients and colleagues.Skills with good work habits, excellent time management and organizational skills.Familiarity with computer operation.Level of general health permitting stooping, bending, climbing stairs and participating in moderately active recreational activities.Physical ability to perform CPR and PMAB skills.Must have and maintain a level of general health which permits the performance of job duties.Positions :
Salary : $45,000-$52,965 Depending on experience and education.
