Social Services Designee / Discharge Planner

The Social Services Designee / Discharge Planner is responsible for assisting and coordinating with the facility's Social Service Department to streamline the process for resident / guest discharge planning.

Responsibilities may include :

Assist the facility with :

Meeting the resident / guest on or shortly after the admission and following them until they discharge to the community.

Attend the 72 hour care plan meeting

Ordering equipment for the community setting

Arranging home health nursing for after discharge, as needed

Coordinating the medications for community use

Coordinate the entire discharge, with IDT involvement

Attending care conferences for sub-acute residents / guests

Attending Medicare Meetings

Assist in procuring any item related to the discharge process for residents / guests anticipated to be returning to the community.

Initiate and coordinate completion of the Post Discharge Plan of Care

Develop an individualized discharge care plan for the resident / guest discharge in collaboration with the facility, the family, and the practitioner, assuring the resident / guest receives the care that will meet their needs in the community.

QUALIFICATIONS

Education and / or Experience :

High school diploma / GED
