Position Details

Earn : $38.54 / hour, plus mileage reimbursement

Schedule : 1st shift hours, Monday-Friday with rotating on-call

The registered nurse (RN) plays a pivotal role in our community-based Dual Diagnosis Treatment Team (DDTT), serving Allegheny County . The dual diagnosis team takes a compassionate, recovery-oriented approach dedicated to providing high-quality care and unwavering support for individuals diagnosed with both a mental illness and an intellectual disability . The RN will spend their time in the community, directly interacting with consumers in their homes, traveling, and documenting their work.

The nurse shares in the responsibility of carrying out rehabilitation and support functions and assisting in treatment, substance abuse services, education, support and consultation to families and crisis intervention services under the clinical supervision of the Program Director and Psychiatrist. The DDTT Nurse is specifically responsible for physical health medical monitoring and education.

Duties and Responsibilities

• Coordinate and monitor medical and psychiatric medications.
• Coordinate medical, vision, dental care.
• Perform periodic health assessments to monitor physical health wellness, medication issues / needs and current fitness and nutritional / dietary needs.
• Develop goals related to physical health wellness, medication management, fitness and nutrition.
• Participate in crisis response on-call schedule on a rotating basis
• Track and manage metabolic screening
• Provide medication education to individuals, family, and supports
