Our client needs an RN Case Manager to deliver field-based hospice care in Cameron County, Texas. This role is part of a growing hospice group expanding across South Texas. They want a steady, compassionate nurse ready to own patient care in the community.

What you'll do

Deliver hospice care in patient homes and facilities

Manage your caseload with clear ownership

Coordinate with care team, patients, and families

Document visits accurately and on time

Rotate on-call (extra pay)

Handle occasional after-hours visits (per-visit pay)

Must-haves

Active RN license in Texas

Hospice experience helpful (not required)

Strong clinical and fieldwork comfort

Reliable and compassionate with patients

Willing to cover on-call and extra visits

$70,000–$80,000 base salary
