Position Summary :

Were looking for a Physician Assistant (PA-C) who believes in person-centered, place-based care . Instead of a rushed clinic setting, youll have time to connect with patients in their own environmentwhere your care can truly change outcomes.

Responsibilities :

• Provide routine and follow-up primary care to older adults
• Collaborate with care teams and support staff on treatment planning
• Monitor chronic conditions and acute medical concerns
• Use EMR to document care clearly and consistently
