We are searching for a talented Community Health Worker III

Hours : 40hrs

Shift : Day shift; 8 : 00am - 4 : 30pm

Location : Southcoast Business Center - Fairhaven, MA; Position will follow a hybrid schedule

Responsibilities

Position reports to the Director, Manager, Team Lead or designee, in collaboration with a multidisciplinary care team, provide culturally appropriate health education, coaching, support, information and outreach in a variety of settings including community-based, primary care, ambulatory, emergency and acute care settings. Identify high risk patients and engage them utilizing effective listening and communication skills to address health and social needs. Utilize critical thinking to navigate systems and facilitate the coordination of care and community resources to meet the patient

Qualifications

Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.

Compensation : Pay rate will be determined based on level of experience.

Pay Range

USD $24.97 - USD $41.60 / Hr.
