Job Highlights :

$10,000.00 Sign on Bonus( 2 Year commitment with SSM Health) for external qualified candidates

The shift is flexible, is either :

4 days-10 hours days or

5 days-8 hours days

Department hour are 7 : 00AM-6 : 00PM-Monday-Thursday. Fridays 7 : 00AM-5 : 00PM

There is a commitment of a weekend rotation -every 5th weekend

Hours are as needed on Saturday and Sunday-Inpatient Patients

Physical Therapist may choose to see the patient in the AM or PM

Out patients Orthopedic with an occasional inpatient as needed.

Waupun Memorial Hospital Physical Therapist staff has many years of experience from which to draw and learn.

We communicate well and offer many services for a Critical Access Hospital including lymphedema, vestibular, dry needling, Functional Capacity Evaluations, and power wheelchair evals.

Job Summary :

Evaluates and conducts medically prescribed physical therapy treatment programs to treat movement dysfunction and pain resulting from injury, disease, disability, or other health-related conditions.

REQUIRED PROFESSIONAL LICENSE AND / OR CERTIFICATIONS

Resuscitation Certification Requirement : Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment. Other resuscitation certifications may be required dependent on department.
