Registered Nurse Case Manager

Position Summary :

The Nurse Case Manager is responsible for coordinating, monitoring and documenting the health care of the children in their assigned caseload. The Nurse is responsible for ensuring that appropriate medical care as required by regulatory oversight and as dictated by good health care practice is provided to all children on our residential campus.

Job Duties :

• Ensure full compliance with internal and external regulatory requirements; obtain health records, birth record, immunization registry and other pertinent medical or mental health information; review ACS triage material and communicate with ACS on as needed basis.
• Actively communicate with community providers scheduling needed appointments and obtaining health related information for the residents
• Provide direct nursing care to residents in the health center with illness or injury.
• Actively participate in obtaining psychotropic and non-routine medical procedures consents when necessary.
• Serve as a liaison between youth and medication prescriber, provide appropriate medication training to youth and family.
• Maintain accurate and up-to-date Medication Administration Records
• Monitor and perform controlled substance counts and destruction of narcotics.
• Participate in medication distribution; communicate medication orders to the pharmacy and check deliveries.
• Communicate and coordinate with clinical team, foster and biological family / guardian about all medical conditions, appointments and / or treatments
• Participate in community visits (e.g. Emergency Room, hospitalizations, community specialty appointments, home visits) on as needed basis.
• Provide formal and informal training for clinical teams, milieu staff, foster and biological parents on specific health care needs of residents including appropriate health practices and administration of medications.

Monday - Friday -9AM -5PM or 8 : 30AM -4 : 30PM

Compensation :

LPN -$30 -$35 per hour

RN -$75K - $92K per year
