Home Health Nurse

Key Responsibilities :

Assess patients in stable / emergent situations

Develop and evaluate individualized Plans of Care (POC)

Document all care activities and maintain accurate records

Educate patients / families on care, medications, and safety

Supervise HHAs / STNAs / LVNs

Perform pain assessments and management

Arrange referrals and monitor third-party compliance

Participate in on-call duties and interdisciplinary meetings

Skills & Competencies :

Required :

Homecare Homebase software experience

OASIS documentation experience

Current RN license in practicing state

CPR certification

Valid driver's license, reliable transportation, and insurance

1–2 years RN experience in clinical care

2+ years home health experience

Preferred : CHPN certification

BSN degree

Education :

Required : Associate Degree in Nursing or Nursing Diploma

Preferred : Bachelor of Science in Nursing (BSN)

• Offered pay rate will be based on education, experience, and healthcare credentials.
