Social Worker Program Coordinator

The VA Eastern Colorado Healthcare System, Social Work Service is currently recruiting for one (1) Social Worker Program Coordinator, GS-0185-12, for Marcus Institute for Whole Health (MIBH) / Avalon Network at the Department of Veterans Affairs Clinic, in Aurora, Colorado. Responsibilities include overseeing the daily operation for the MIBH / Avalon Network / VHA partnership and programs, developing policies and procedures for program operation. The position also involves program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. The role includes meeting with Veteran / Service Member and family / caregiver providing direct patient care, education on VA health benefits and resources, and case management and care coordination. The Social Worker will utilize and / or refer to both VA and non-VA resources across the continuum to address current medical, mental health, and psychosocial needs. Collaboration with interdisciplinary teams across VHA health care systems to facilitate care coordination is also required. The position involves preparing reports, briefs, and presentations for VA staff at all levels, MIBH / Avalon staff, Congressional Staff, Community Organizations, etc., regarding the MIBH / Avalon VA process and specific mechanics of the program. Independently managing the day-to-day operations of the MIBH / Avalon Liaison initiative and providing accountability to program effectiveness and modifications of service patterns to enhance customer service are also part of the role. The Social Worker will identify gaps in processes and collaborate with MIBH / Avalon and VA to enhance these processes and procedures. The position requires providing analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. Additionally, the Social Worker will initiate and conduct service audits and complete designated clinical practice audits and reports, including productivity assessments. Developing, gathering, and applying new data as needed to successfully plan for and implement program goals and projections is also required. The Social Worker will develop and implement transition policies and programs that are of vital significance and interest to the top management of VHA, VA, RMR / MIBH, Veterans Service Organizations, and Congress. Providing leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff is also part of the role. The work schedule is Monday-Friday, 8 : 00am-5 : 00pm.
