RN Home Health Clinical Manager

We are working with our client a nonprofit home health and hospice agency in the Montpelier VT area. We are working to fill a Home Health Clinical Manager to oversee the home health area of the agency.

Position Overview :

As the RN Home Health Clinical Manager you will oversee and manage the clinical operations . You will be responsible for ensuring the delivery of high-quality patient care maintaining compliance with all regulatory standards and managing a team of nursing staff.

Key Responsibilities :

Oversee and manage all clinical operations of the home health agency including patient care and staff management

Develop and implement policies and procedures to ensure compliance with all state and federal regulations

Monitor and evaluate patient care to ensure quality and effectiveness

Provide guidance and support to nursing staff including training and development opportunities

Collaborate with other members of the healthcare team to develop and implement individualized care plans for patients

Conduct regular performance evaluations and provide feedback to nursing staff

Manage the coordination of patient visits

Maintain accurate and complete patient records

Participate in the hiring and onboarding process for new nursing staff

Stay up-to-date on industry trends and best practices to ensure the delivery of high-quality care

Qualifications :

Current RN license in Vermont

3 years in nursing

2 years of experience in home health

Previous management experience preferred

Strong knowledge of state and federal regulations related to home health

Excellent leadership and communication skills

Ability to multitask and prioritize in a fast-paced environment

Proficient in electronic medical records and other computer systems

Strong problem-solving and critical thinking skills

Ability to work independently and as part of a team
