Occupational Therapist (Per Diem) – Addington Place at College Harbor

We are currently seeking a licensed Occupational Therapist (OT) to join our dedicated rehabilitation team on a per diem basis. This position offers flexible scheduling in a positive, collaborative environment focused on resident success and well-being.

Key Responsibilities :

• Conduct comprehensive evaluations to assess residents’ functional abilities and daily living skills.
• Develop and implement individualized treatment plans to promote independence in self-care, mobility, and meaningful activities.
• Provide skilled occupational therapy interventions focused on restoring function, enhancing safety, and improving quality of life.
• Collaborate with nursing, physical therapy, and speech therapy staff to ensure a coordinated, holistic approach to care.
• Educate residents, families, and caregivers on adaptive techniques, home safety, and use of assistive devices.
• Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines.
