On An Average Day You Will :
Includes, but not limited to)
Receive case referrals and review available patient information related to the case, including home visits, to determine hospice / home care needsAssign appropriate hospice personnel to referral case as neededConfer with Medical Director and Attending Physician regarding any questions as to an individualâ?™s eligibility for servicesReview and evaluate each case through a variety of means such as home visits, conferences, records review, and the services provided by cliniciansReview patientâ?™s medical diagnosis, prognosis, medications, procedures and clinical courseDiscuss and verify impressions, instruct and guide clinicians to promote more effective performance and delivery of quality home care servicesAvailable, during operating hours, to assist clinicians as appropriateConduct clinical record reviews and communicate findings and recommendations to the Clinical Director and hospice personnel, as defined by the QAPI ProgramAssist in the screening and interviewing process of new employees and make recommendations for employment of individualsAssist in the orientation of new employees, as neededAssist the Director of Patient Care Services and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programsAssist in the formulating, revising, implementing and evaluating organization policies, procedures, goals and objectives â?? both short- and long-rangeComply with accepted professional standards and principlesAssess the health needs of the community and assist in the development of plan(s) / program(s) to meet needsParticipate in public relation and community activities that promote the organizationâ?™s role as an effective member of the hospice and health care delivery systemPromote customer service orientation to all hospice personnelParticipate actively in quality assessment performance improvement teams and activities as directed by the Director of Patient Care Services in accordance with the QAPI ProgramPerform other duties and activities as delegated by the Director of Patient Care ServiceAdditional duties as assignedQualifications :
A Registered Nurse license in good standing to practice professional nursing in the stateMust have previous experience in a hospice home care setting with two years of management or supervisory experienceAn enthusiasm for providing the best possible care to patients and their familiesUnderstanding of the hospice philosophy and issues of death and dyingExcellent observation, verbal, and written communication skillsA valid driverâ?™s licenseCurrent CPR certificationWe Got the Perks :
