Medical Director - PACE Program - West Springfield, MALocation : West Springfield, MA - Hartford 25m, Boston 90mPosition Type : Full-Time, Permanent Overview : A community-based Program of All-Inclusive Care for the Elderly (PACE) organization is seeking a dedicated and experienced Medical Director to provide clinical leadership and oversight for its innovative model of comprehensive senior care. This role offers the opportunity to guide an interdisciplinary team in supporting older adults with chronic care needs, helping participants maintain independence and quality of life while living safely in their communities. Full-time, leadership position Monday through Friday schedule Occasional weekend or on-call participation as required Reports to the Executive Director Practice Setting : Community-based PACE program (outpatient and in-home care) No locum or visa candidates accepted Responsibilities : Provide general medical direction and supervision for participant care across all service settings Conduct new participant history and physicals, periodic reassessments, and event-based evaluations Develop and oversee individualized medical treatment plans Supervise and support primary care and advanced practice providers within the program Lead the Quality Assurance and Performance Improvement (QAPI) program Develop and maintain medical policies, procedures, and care protocols Collaborate with the interdisciplinary team to optimize participant outcomes and care coordination Participate in intake assessments and care planning discussions with participants and caregivers Educate participants, families, and team members on health maintenance and chronic condition management Provide oversight for compliance with state, federal, and regulatory standards Requirements : Active Massachusetts medical license (plenary physician license) Board Certified in Internal Medicine or Family Medicine Board Certification in Geriatrics preferred DEA registration and CDS certification required Minimum 1 year of experience working with frail or elderly populations Previous supervisory or leadership experience required Strong skills in physical assessment and chronic disease management for older adults Demonstrated ability to manage changing priorities and participant needs Desire to work in an innovative, community-based healthcare model Compensation and Benefits : Competitive salary based on experience Health, Dental, and Vision Insurance Retirement plans Paid Time Off CME support Life and Disability coverage Professional development opportunities within an expanding care model
