Clinical Program Coordinator

We are searching for a Clinical Program Coordinator someone who works well in a fast-paced setting. In this position, you will coordinate all activities related to the clinical program. This includes but is not limited to : coordination of all activities related to evaluation and consultation, coordination of Physicians / OR teams for surgical interventions, family communication and education, coordination of multidisciplinary team to assess and care for the patient, care of the inpatients, discharge planning, outpatient follow-up as well as pertinent data management for the program / clinic for research, outcomes and / or reporting purposes.

Responsibilities

• Provide education to those in need
• Promote education about the program to physician groups as well as in the community as observed by clinic chief or designee
• Coordinate all activity necessary for the program / clinic
• Assure that all patient care and testing are coordinated with consultants and primary care physicians
• Provide follow-up on patients
• Coordinate interdisciplinary team conferences to review cases and determine agreed upon plan of care
• Act as a liaison between the physicians and other staff
• Establish and maintain good working relationships with patient, patient support system, peers, physicians and other hospital departments
• Maintain a database of potential patients
