Minimal call responsibilities

Lead your care team, collaborating with experienced Advanced Practice Providers

Support from an extensive in-practice team consisting of behavioral health, social work, pharmacy, nutrition, care coordination, and rehab resources .

Locally governed, physician led, professionally managed and community focused program ensures autonomy and a voice in your practice

Epic EHR

M to F 8 am to 5pm

On call 2 to 3 weeks per year for Monday through Sunday.

What You Will Need

MD or DO

Active / unrestricted medical license

Board certified / eligible

Family Medicine or Internal Medicine trained with a passion for senior care and population health.
