Major / Essential Functions

• Plan, coordinate, and implement GUIDE program initiatives and events to engage rural healthcare professionals and promote dementia care, awareness and services.
• Coordinating assessments within GUIDE program of patients.
• Develop and manage relationships with healthcare professionals, organizations, community groups, and government entities to support and provide GUIDE program initiatives.
• Assist in developing, managing / maintaining a database of contacts and resources for GUIDE program.
• Perform other duties as assigned in order to accomplish the mission / goals of GUIDE program and the GIA.
