We are seeking an experienced Nurse Practitioner or Physician Assistant with expertise in Physical Medicine and Rehabilitation (PM&R) or orthopedics to join our team in Ft. Collins, CO. Our private practice group provides geriatric care services to patients in skilled nursing facilities throughout the area. As our Nurse Practitioner or Physician Assistant you will play an essential role in improving the quality of life for our patients by providing top-notch physical medicine and rehabilitation services.

Responsibilities :

• Conduct comprehensive physical assessments, consultations and follow-up visits
• Create individualized treatment plans for patients in need of PM&R services
• Develop and implement therapeutic exercise and rehabilitation programs
• Manage medication and pain management strategies
• Collaborate with physicians and multidisciplinary rehabilitation team to optimize patient outcomes
• Educate patients and their families
• Maintain accurate patient records and documentation
• Provide outstanding care and coordinated rehabilitation effort within skilled nursing facilities under the supervision of a PM&R physician
