• Remote role with up to 75% field based work.

Candidate must reside in one of the following counties within the state of Iowa : Black Hawk, Dallas, Des Moines, Freemont, Guthrie, Henry, Jasper, Jefferson, Johnson, Lee, Mahaska, Marion, Marshall, Page, Polk, Pottawattamie, Tama, Taylor, Warren, Washington, and surrounding areas.

Position Purpose : Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans / service plans for long-term care members and educates members and their families / caregivers on services and benefits available to meet member needs.

Education / Experience : Requires a Bachelor's degree and 1 year of related experience.

Or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position.

For Iowa Plan Only : Bachelor's degree required and 1+ years of experience with populations served, or RN with 6+ years of experience with population served.

Pay Range : $22.50 - $38.02 per hour
