Role Description

This is a full-time, on-site MDS Coordinator role based in Spokane, WA. The MDS Coordinator will oversee the development, implementation, and monitoring of care plans to ensure residents are receiving optimal care. Day-to-day responsibilities include completing assessments, maintaining compliance with regulatory guidelines, collaborating with healthcare teams, and utilizing the Minimum Data Set (MDS) system to ensure accurate reporting. The role requires hands-on care planning and direct interaction with residents and staff to uphold quality standards in geriatric nursing.

Qualifications

• Current Licensed Practical Nurse (LPN) or (RN) certification in the state of Washington
• Knowledge and experience with MDS and Care Planning processes
• Proficiency in creating and managing individualized Care Plans
• Experience and skills in Geriatric Nursing; understanding of elder care best practices
• Strong communication and teamwork abilities with a focus on collaboration
• Prior experience in long-term care or skilled nursing facilities is highly preferred
• Attention to detail, regulatory compliance knowledge, and excellent organizational skills
