Position Overview

HumanEdge Health is seeking a Travel Physical Therapist Assistant (PTA) for a home care assignment in Mansfield, CT . This role provides outpatient and home-based physical therapy services, supporting patients in maintaining mobility, strength, and independence.

Responsibilities

Provide physical therapy interventions in patients’ homes according to the plan of care

Assist with exercises, mobility training, and functional activities

Monitor patient progress and report outcomes to the supervising PT

Educate patients and families on exercises, safety, and home modifications

Maintain accurate documentation in compliance with agency and regulatory standards

Collaborate with interdisciplinary teams as needed

Qualifications

Current Connecticut PTA License required prior to start

Minimum of 1 year home care experience preferred

Strong interpersonal and communication skills

Ability to work independently in home care settings

Reliable, professional, and compassionate approach to patient care

Shift Details

Monday – Friday, 08 : 00 AM – 04 : 30 PM

40 hours per week

No on-call responsibilities
