JOB SUMMARY : We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our dedicated team at our Federally Qualified Health Center (FQHC). As an LPN, you will play a vital role in providing comprehensive healthcare services to our diverse patient population. PRIMARY DUTIES AND RESPONSIBILITIES :

• Patient Care and Assessment :
• Conduct initial patient assessments, including vital signs, medical history, and current health status.
• Administer medications, immunizations, and treatments following established protocols.
• Monitor patient progress, provide health education, and offer support and counseling as needed.
• Collaborate with healthcare providers to develop and implement individualized care plans.
• Clinic Support :
• Assist healthcare providers with examinations, procedures, and minor surgeries.
• Prepare and maintain medical equipment, ensuring their functionality and cleanliness.
• Maintain accurate and up-to-date patient records, documenting all interactions and interventions.
• Perform basic laboratory tests and specimen collection as required.
• Coordinate referrals to other healthcare professionals or community resources when necessary.
• Patient Advocacy and Education :
• Advocate for patients' rights, needs, and preferences, ensuring their voices are heard and respected.
• Educate patients and their families about health conditions, preventive care, and self-management techniques.
• Promote healthy lifestyles and disease prevention strategies within the community.
• Collaboration and Teamwork :
• Collaborate effectively with multidisciplinary teams, including physicians, nurses, and support staff.
• Participate in team meetings, case conferences, and quality improvement initiatives.
• Foster a positive and supportive work environment, promoting open communication and teamwork.

OFF-SITE WORK :

Although it is not typical, Off-Site Work could, at times, be required of this position. This off-site work will typically include, but is not limited to, performing nursing duties (i.e. Vital Signs) at Health Fairs or other HOPE Sponsored Events and attending off-site meetings, trainings or conferences.
