Summary of Position
The Senior Practice Director (SPD), Chronic Care Management, is responsible for the overall practice operations of Chronic Care services. This role supports the strategic execution of Chronic Care Management (CCM) service line initiatives, including leadership and individual contributions over its people and processes. Through collaboration with various departments across Integrated Care Delivery, Product and Engineering, Marketing, Sales, Client Success, Client Implementation, Data Science, and Analytics, the SPD is responsible for timely delivery of CCM business objectives, addressing barriers that may inhibit successful execution of the service, identify and driving proactive solutions to position the long-term success of the CCM program and organizational goals.
The Senior Practice Director drives capabilities and processes supporting growth of the product by partnering with cross functional leadership on strategic initiatives geared toward improving market position, operational efficiency, patient experience, and clinical outcomes. This role requires decision making that will materially affect the overall division and the organization's financial objectives, performance objectives, and strategic results.
Essential Duties and Responsibilities
Operational Management : 50%
Oversee the daily operations of the virtual healthcare team (including health coaches, registered dieticians, nutritionists, nurses, medical assistants, and other support personnel) to ensure efficiency and effectiveness.
Provide active leadership to the operations team by fostering accountability, professionalism, and collaboration both intra and interdepartmentally.
Design and maintain performance tools and reporting systems to track key metrics. Analyze data trends to assess operational effectiveness, identify gaps, and recommend strategies for continuous improvement.
Advance operational business design to continually improve the products and services offered to patients.
Collaborate in the development and execution of strategic plans that drive practice growth, strengthen financial performance, and elevate service quality.
Staff Management : 25%
Recruit, onboard, and develop staff to build and sustain a high-performing team. Provide ongoing coaching and performance management to ensure team success.
Provide leadership and support to staff, ensuring adherence to best practices, compliance and regulatory requirements. Develops and complies with appropriate policies to be evaluated and maintained.
Stakeholder and Service Line Management : 25%
Collaborate with the Service Line General Manager and internal teams to support growth of the product and continuous improvement initiatives.
Foster a positive work environment through proactive communication and problem-solving.
Lead cross-functional projects from initiation to completion, ensuring timelines, budgets, and quality standards are met.
Drive standardization of processes and communications to ensure alignment with departmental goals with broader organizational impact.
Promote innovation to improve workflows and successful CCM delivery.
Monitor financial performance and implement corrective actions as needed.
Other duties, as needed.

Qualifications Expected for Position
Master's Degree preferred in MBA, MHA, MPH, MSN, MSW,or other advanced degrees in healthcare, business, or nutrition-related fields.
Demonstrated ability to effectively lead change in a fast-paced growing organization.
Excellent oral and written communication skills.
Ability to function in a fast-paced, high-pressure environment and to be able to meet deadlines under extreme pressure.
Well-organized self-motivator with a high Say-Do ratio.
Outstanding team player with strong interpersonal skills. Interacts effectively with staff, employees, leaders and providers.
Ability to generate, analyze, and summarize reports and documentation accurately and efficiently
Ability to work remotely, with the flexibility for on-call responsibilities and travel, as needed.

Bachelor's degree required in a relevant field such as Healthcare Administration, Nursing, Nutrition, Dietetics, Public Health, or related discipline.
7+ years of operational leadership in a multi-specialty practice and or virtual care setting.

The base salary range for this position is$140,000 - $180,000. In addition to a base salary, this position is eligible for a performance bonus and benefits (subject to eligibility requirements) listed here : Teladoc Health Benefits 2026 .Total compensation is based on several factors including, but not limited to, type of position, location, education level, work experience, and certifications.This information is applicable for all full-time positions.
