About the Opportunity

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our clinical team for an 11-week contract. This role is ideal for an LPN who thrives in a supportive, patient-centered environment and enjoys providing hands-on care in a rehabilitation setting.

You'll work closely with other healthcare professionals to deliver exceptional care to residents while maintaining high standards of safety, documentation, and professionalism.

Responsibilities

• Provide direct nursing care in accordance with the facility's policies and state regulations
• Administer medications and treatments accurately and on schedule
• Monitor and document residents' conditions and report any changes to the supervising RN or physician
• Assist with daily living activities and ensure residents' comfort and safety
• Collaborate with interdisciplinary care teams to develop and implement care plans
• Maintain accurate records in EMR systems and uphold confidentiality standards
• Support infection control and quality assurance initiatives
