We are seeking a dedicated and compassionate Physical Therapist or Occupational Therapists to join our team for a home care role. The ideal candidate will have a passion for patient care and the ability to work independently, driving from patient to patient to provide physical therapy services in the comfort of their homes.

Key Responsibilities :

• Patient Care : Provide high-quality physical therapy services, primarily focusing on routine visits with some assessments.
• Assessment and Planning : Conduct thorough evaluations to determine patient needs and create personalized care plans.
• Therapeutic Intervention : Implement and monitor therapy programs designed to improve mobility, reduce pain, and restore physical function.
• Documentation : Maintain accurate records of patient progress, treatment plans, and outcomes.
• Communication : Collaborate with healthcare providers, family members, and caregivers to ensure coordinated care.
• Patient Education : Educate patients and their families on exercises and activities to enhance therapy outcomes and promote long-term well-being.
• Travel : Travel between patients' homes within the designated area, adhering to a schedule that meets both patient and company needs.
