Telehealth Neurology Nurse Practitioner

Focused on neurodegenerative diseases, our client partners with neurologists and clinics across the Bay Area to deliver personalized, at-home support through Principal Care Management (PCM) and Chronic Care Management (CCM) programs.

Salary :

$170,000 to $210,000 (negotiable based on experience)

Job Summary :

Our client is seeking an experienced Nurse Practitioner (NP) specializing in neurology and chronic care management (PCM / CCM) to provide proactive, patient-centered care for individuals with neurodegenerative diseases such as Alzheimer's, Parkinson's, dementia, and mild cognitive impairment (MCI). This role is based in the San Francisco Bay Area with flexibility for hybrid or remote work, depending on patient and clinic needs.

Job Responsibilities :

• Deliver telehealth follow-up care for patients with chronic neurological conditions.
• Develop, document, and maintain care plans compliant with Medicare PCM / CCM requirements.
• Collaborate with neurologists and healthcare providers to ensure seamless care coordination.
• Lead and support Medical Assistants to ensure clinical excellence and teamwork.
• Communicate effectively with patients, families, and caregivers to promote treatment adherence.
• Record and submit accurate time-based activity logs for compliant CPT billing.
• Partner with the care operations and AI teams to enhance workflows and care processes.
