Senior Population Health Specialist

A company is looking for a Senior Population Health Strategy Specialist.

Key Responsibilities

Develops and designs complex population health strategy initiatives and operational plans to improve care coordination and health outcomes

Manages and tracks clinical, quality, documentation, and data submission projects to achieve strategic objectives

Monitors care management trends and analytics to support program initiatives and identify high-risk member populations

Required Qualifications, Training, and Education

Bachelor's degree and 4-6 years of related experience, or equivalent experience

Knowledge of case management and utilization management

Experience in project management and process improvement

Exceptional computer and analytical skills

Familiarity with Kentucky Medicaid regulations
