Job Summary :

In this role you will provide routine, individualized personal cares to Residential Hospice patients. You will monitor and report changes in patient health status to the Case Manager. Also assist with basic residential household activities.

Key Position Details :

• 0.8 FTE - (64 hours per two-week pay period)
• 8-hour weekday evening / night shifts (3pm-1130pm & 11pm-730am)
• Every 3rd weekend 12-hour evening / night shifts (7pm-730am)

Job Description :

The Home Health Aide provides routine, individualized personal cares to Home Health, Hospice and Residential Hospice patients. Monitors and reports changes in patient health status to Case Manager. Also assists with basic residential household activities.

Principle Responsibilities

Delivers care that meets or exceeds patient expectations based on patient / family feedback.

Documents all patient visits to reflect cares given during the visit or shift, patient response and safety interventions that are in place.

Pay Range

Pay Range : $19.18 to $26.12 per hour

The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and / or other differentials may be available to increase your pay rate for certain shifts or work.
